Position: Community Health Worker
Job Summary:
The Community Health Worker is responsible for providing health education, support, and outreach services to individuals and communities within the assigned area. This role focuses on promoting public health and enhancing community well-being through various outreach activities and support programs. The Community Health Worker will collaborate with healthcare professionals, community organizations, and local stakeholders to address health disparities and improve health outcomes.
Responsibilities:
1. Conduct community assessments to identify health needs, social determinants of health, and barriers to accessing healthcare services.
2. Develop and implement culturally sensitive health education programs, workshops, and activities to promote healthy behaviors and prevention strategies.
3. Provide individual and group support to clients, including counseling, advocacy, and referrals to appropriate health and social services.
4. Collaborate with healthcare providers, community organizations, and local agencies to facilitate access to healthcare resources and services.
5. Organize and participate in community health promotion events, health fairs, and awareness campaigns.
6. Collect and maintain accurate records of client interactions, services provided, and program outcomes.
7. Conduct home visits and outreach activities to engage individuals and families in health-related initiatives.
8. Assist in the development and implementation of public health initiatives, such as vaccination campaigns and disease prevention programs.
9. Stay updated on current health trends, public health policies, and evidence-based practices in the field of community health.
10. Collaborate with interdisciplinary teams to develop and implement strategies for addressing social determinants of health at the community level.
Required Skills and Qualifications:
1. Bachelor's degree in Public Health, Social Work, or a related field.
2. Proven experience in community health work or related roles.
3. Extensive knowledge of public health principles, community health promotion strategies, and social determinants of health.
4. Strong understanding of cultural competency, health equity, and social justice principles.
5. Excellent communication skills, both verbal and written, with the ability to effectively engage diverse populations.
6. Demonstrated ability to build and maintain positive relationships with community members, stakeholders, and healthcare professionals.
7. Knowledge of community resources, social services, and healthcare systems.
8. Ability to work independently and as part of a team, while managing multiple priorities and deadlines.
9. Proficient computer skills, including Microsoft Office Suite and data management systems.
10. Demonstrated commitment to maintaining client confidentiality and adhering to ethical standards in the provision of services.
